There is no need to include patient details. We can work from a description — the form, the box, what you expected and what happened instead. We would rather have that than a record.

If something looks wrong on the form itself
Some problems are almost impossible to put into words — text running past the edge of a box, a tick landing between two options, a line cut off at a page break. You do not have to describe those. Switch to the practice patient and try the same form on her.
She is a made-up record built to be awkward on purpose: a long hyphenated surname with an apostrophe in it, an employer name that runs the width of the page, an accented letter, a leap day. Most problems that show up on a real patient show up on her too — and her name reads PRACTICE PATIENT — DO NOT FILE, so a sheet printed from her is unmistakable. You will find her in the app: start a new form, choose the one that went wrong, and pick the practice patient instead of a real one.
Once it goes wrong on her, there is nobody's information on your screen, so it is safe to show us. The live look happens on your equipment, not ours — point your phone camera at the monitor while we talk, or start a screen share from your practice's own software and let us in as a guest. We do not run a screen-sharing tool, we will never ask to see a real patient's record, and if one appears we will say so and look away.
If a problem will not happen on the practice patient, describe it in words instead — the form, the box, what you expected, what appeared. That is genuinely enough more often than it sounds.
Three things worth trying first
Most of what goes wrong is one of these, and all three are faster than waiting for us.
- The licence email never arrived. Check the spam folder, and check the address you typed. Practice mail systems quarantine unfamiliar senders, and the message comes from a name you have not seen before.
- The key will not paste. Select the whole thing — it is long, and a copy that stops at the first line break looks complete on screen. Paste it into a plain text box first if you are unsure.
- Nothing loads on the clinic machine. Try the same address on your phone. If it works there, the practice network or an installed extension is blocking it, and that is worth knowing before you write to us.
